Abortion ICD9 and CPT4 Codes and Maternity Medical Billing
By Angel Bean
This information on filing medical billing claims is intended to help medical billing professionals and is not intended for patients or the general public. Abortions and maternity cases are handled in a special way especially if the woman had a miscarriage or terminated her pregnancy even if the woman had the baby we handle these cases with a little more compassion on the phone as sometimes they are courtesy cases or written off or we take the insurance as payment in full. The code for an incomplete abortion is 634.91 and the code for dilitation and curettage for an incomplete abortion is 59812. In this case you would try to get as much money in a payment plan from the patient then if they don't want to pay write it off based on the procedure and the monies received. However you may disagree with the policy for writing this off in your practice. The anesthesiology code of 01967 with a base of 5 or 01968 with a base of 3 are maternity codes for new mothers. When it comes to maternity bills some doctors allow their patients to be in pain and under the epidural or anesthesia for a long time to get a high amount of money. However some doctors stop the anesthesia after a certain time and give the woman a c-section which would then be medically coded a failed trial of labor then c-section. However, not all doctors cut the labor at a certain point and then split the difference with the patient. Some doctors give courtesies, some Manhattan, NY doctors whose bills are high anyway don't so each doctor's group is different. Pain management claims after the mother has a C-section are separate claims that don’t need the anesthesia code time or units. Sometimes the medical biller has to reduce the bill because we can't bill the patient for the co-insurance. Never tell the patient or the insurance company that we are writing off a balance. In this special scenario if you write off an account and then later on the carrier does pay something on it you can always reverse the decision to write off the account and then accept the insurance as payment in full. This way the medical billing company at least gets something before we have to write it off. If a customer calls in and mentions that they are pregnant now casually ask them how far along they are in their pregnancy and then if they are between four to six months pregnant you can advise them to contact their insurance company to add the baby to their health insurance policy while they are still pregnant. What happens is if you wait until the baby is born to add the baby to your insurance policy, then some medical insurances may not cover the baby or the maternity costs. This is especially true if the patient gets her medical insurance as part of a plan from her employer. The ideal time would be to give the insurance company three to four months lead time to add the baby to the insurance policy before the baby is born. If the patient had more than one office consultation per year according to their medical contract for example, if the patient consults with a heart doctor and then an OBGYN the patient is not responsible for this without CID. I think CID refers to chronic illness and disability. The patient is responsible to know their medical contract and what is covered and some policies state only one office visit per year will be covered under their insurance policy.
